As a member of EPO Plus, you are entitled to one routine eye examination per twelve month period.  GEMCare and DRMG have each contracted with several local optometrists to provide these services on a self-referral basis.  Please see the respective network specialist listings for GEMCare and DRMG for the panel listing.

To request an authorization for a routine eye exam:

• Make an appointment with one of the contracted optometrists.  Have your health plan identification card available.
• Your appointment should be scheduled after five working days to allow time to verify eligibility and benefits.
• The optometrist's office will coordinate an authorization from Managed Care Systems, administrators for GEMCare and DRMG.

This authorization covers the routine optometry examination only.  If you have questions about your eyewear benefit, please contact the CHW Bakersfield Benefits Coordinator.
