Welcome to EPO Plus.  Included in this website are lists of provider networks, including primary care physicians and network specialists, as well as services offered to CHW employees and their dependents. 

EPO Plus offers two choices in provider networks, GEMCare Medical Group and Delano Regional Medical Group (DRMG).  Please review both lists and determine which one best meets your geographic needs.  After choosing a medical group, select a primary care physician for yourself and your dependents.
